This was a case of double encoding. 

The problem is that the column title was encoded with escape() before being put in a form hidden field. At the point, "Street name" became "Street%20Name". Because the form itself is www-url-encoded, "Street%20Name" became "Street%2520Name" on the submission of the form. When the web server received this string, it decoded it to "Street%20Name" which is what appeared in the UI.

The bottom line is that strings put in form should not be encoded for URI.
