The fix was done by Jason Nogar. Basically, we added a return value to the ProcessMarker API call. It's a bounding box that is optional and can be set by the Renderer::ProcessMarker call to contain the actual bounding box of the symbol, when insertion point is taken into account. Once this is known by the PointAdapter class, which determines the positions for the candidate labels for the point feature, it can do a better job calculating these and placing labels closer to the symbol. The fix also includes better handling for rotated symbols -- labels are now closer to the rotated symbol.
