Ah, you are using MBTiles and it looks like the tile removal part only works for filebased caches.

I'v created a ticket: https://github.com/mapproxy/mapproxy/issues/9

For now you should try to remove the broken tile from the .mbtiles file by hand or recreate the whole cache.
