Motion: Rename map.h to mapserver.h

Thanks for the quick vote. I have committed the change to rename map.h 
-> mapserver.h to SVN a few minutes ago, including a mention in 
MIGRATION_GUIDE.TXT. Everything seems to build fine on the buildbots 
that I tried, so hopefully I didn't miss any file.
