Thanks to some help from Andy Colson, I have profile output to report.
I compiled mapserver with --with-gd=static,/usr and ran shp2img -m test2.map -o image3.jpg with "ANGLE FOLLOW" in the mapfile and the same command with "ANGLE AUTO".

Angle auto completes on my system in 1.6 seconds where angle follow completes in 5.1 seconds.  

Attached are the corresponding profile reports.

Chris,

Maybe I missed this, but can you state your shp2img draw time results for MapServer 5.4.2 on Windows and Linux for your test dataset?  Are they the same?  If yes, then are you saying that the problem is with MapServer 5.4.2 only?
