[mapserver-dev] Mixed code and variable definition breaks MSVC builds

I've fixed some last places where these warnings still appeared, along
with quite a few other compiler warnings. If not for one warning in
maplexer.c (which we can't really intervene on as it's generated) and
one in the AGG headers, we could now add -Werror :(
