Log file fields?

Hi,
  I'm curious what each of the fields are within the log file.
Here's my guess:
  Date
  ?
  IP address
  Application
  0/8 (0-browse, 8-query)?
  Extent
  Visible Layers
  ?

  It'll show the visible layers, but is there any way to tell which layer is
being queried? (I'm getting a "getNumeric(): Incorrect data type" error
people query, but I'm not sure which layers is causing it).
  I'd appreciate any help you could give me.
