[mapserver-users] source files of gmap

Looks like your web server desn't recognize .phtml as a PHP file
extension and just returns the page source directly without running it
through the PHP parser.

Find the line with "AddType ... .php" in your httpd.conf and add a
similar line for ".phtml"... that should do the trick.
