You probably have the mapscript.dll also loaded by PhP by default.
Check your php.ini.

There are two options:
1. To load mapscript.dll with php by default (uncheck it in the list of
extensions in php.ini)
OR
2. To load it at the execution time (when needed) the way it is in your
script.

Both at the same time do NOT work, since obviously double registration
of functions is not possible.
