I know it's not much consolation since it's far from being very usable
... but Scott Ellington is working on a pretty promising project:

I haven't gotten involved as I had hoped due to time constraints, but
I do hope to eventually.  The plan is for Appomattox to have an API a
la MonoDevelop and making a plug-in for MapFile export should be a
breeze.
