Reference map with mercator projection

Can anyone give me an example of using a reference map with a mercator
projection?  I've managed to get reference maps working well with
unprojected maps of the world, but I'm having problems with projected
reference maps.

I'd be interested in seeing mapfiles and any associated
code/images/stuff.  May maps need to display the entire world.
