with MapServer itself it is not possible. You'd have to pre-process the 
data with a tool that can do that. We used MapInfo and SmartLabel 
(www.maptext.com) to achieve what you want. See the results on 
http://www.ortsplan.ch/home/en/index.html (search i.e. for Chur and 
zoom in).
