Frank,
I just want say you Thanks.
setting CONFIG "CPL_DEBUG" "ON"
and using ogrinfo and shp2img on the same boundary box I have been able to
identify the error and resolve it.
There was some corrupted GIF images and GDAL was not able to generate any
images when one of these was involved in my WMS request.
