Nick,
in mapcairo.c near line 857, function

int saveImageCairo (imageObj *img, mapObj *map, FILE *fp, outputFormatObj
*format)

try changing fwrite to msIO_fwrite

In any case, please open an issue for this one so it doesn't get forgotten.
