 Mapserver has backported WFS paging to WFS version 1.1, by using
 startPosition and maxFeatures.

 GeoServer is on the verge of doing the same, however might choose
 startPosition and count (exact same parameter names as WFS 2.0).

 So it makes sense for our writers to be prepared for this and output
 startPosition and count if specified in the options.
