[pdal] C# files missing in GitHub

Okay there are no files missing.  I have taken it down to the SWIG doesn't like that boost multi index container that the new implementation of Dimensions is using.  It is making a SWIGTYPE whose name is too long to save in Windows, thus not allowing the first build to find the file because it could not be created and it all trickles down from there.

Which files?

(The C# files are swig-generated, so I suspect they're not missing - just not being built. I'll check this weekend.)

It appears that files necessary to the c# solution are missing from github.  Anyone know where I can get them?
