[postgis-devel] PostGIS documentation on refractions.net website

I've been cleaning those up as I come across them.  Mostly I've been trying to fill in examples where its not intuitively obvious how to use a function and where it is commonly useful to use it or at least I have had a need to use them.

Paul, that's great, thank you very much! Now I feel inspired enough to
start committing some documentation changes relevant to SVN HEAD :)

Incidentally if anyone else is interested in some more doc cleanups
(Regina?), I notice that some of the examples in the existing
documentation still use the non ST_ prefix function names.
