 Since introduction of -m switch in pgsql2shp (ticket #885) it would be
 nice to have an -m switch in shp2pgsql, for simmetry.

 To be extra nice, the _same_ mapping file should be used,
 which means reading DBF names on the second column and db attribute names
 on the first column.
