 You may actually have found the culprit. Re-add that include and try it.
 I'll be off for ~ 7 hours...

 If confirmed we should move the PREPARED_GEOM define from that include to
 a postgis_config.h or something like that (driven by configure) to reduce
 future cases of this.

 Beside, if this is confirmed it means prepared geoms will only be
 available
 in postgis, not in liblwgeom.
