 The damage occurs near a very tiny hole in your polygon:

 [[Image(nei_hole.png)]]

 [[Image(nei_hole_big.png)]]

 The distance between the two closest red points there is around 1.6,
 which is bigger than your tolerance, so that doesn't need to be the
 reason...
