I filed a bug in ShapeLib's bug tracker:

w.r.t. _FILE_OFFSET_BITS nonportability, to be less intrusive, instead of
specifying -D_FILE_OFFSET_BITS=64 in the Makefile's CFLAGS, we could just
#define _FILE_OFFSET_BITS 64
at the top of pgsql2shp.c, before any #includes as described here:
