Thanks. I have now found the information I was looking for, and I think
it should be:

+proj=tmerc +k=0.9996 +x_0=500000 +y_0=0 +lon_0=15.0 +lat_0=0.0
+towgs84=0,0,0,0,0,0,0 +no_defs

Using my settings, I match the SWEREF TM sample data (provided on that
page) exact.
