[PROJ] Reviews requested: On GDA2020, PROJ 6 and QGIS

Nyall, I am on vacation right now, and have not had time to read all your
work (although at a first glance, it looks excellent). I have, however,
added suggested changes correcting the actual  chronology of PROJ
development (pipelines and geodetic building bricks arriving in PROJ v 5,
and the results of the GDAL barn raising in v 6).

I have also added references to the two conference papers introducing the
pipeline operator: Kristian Evers did the presentation for the surveying
crowd during FIG working week, 2017, while I introduced pipelines to the
geodesy/geoinformatics crowd during European Geosciences Union General
Assembly, also in the spring of 2017.

I have allowed myself to put the reference to the "Knudsen and Evers" paper
before the "Evers and Knudsen" one. This is solely to assert my academic
priority as the designer and implementer of the PROJ pipeline concept.
While unimportant to my current "operational geodesist"-role, it is of
importance to my prior (and perhaps future) "research geodesist"-role. Feel
free to change the order if this seems like too much vanity for an
otherwise perfectly fine day :-)
