Hello everyone,
I have designed the questionaire and is not limited to any number of
questions and can be found here <http://www.jotform.co/form/50573680504858>,
this is linked to a googledoc spreadsheet that anyone can see (or edit if
you want me to do that, will need gmails) here

I added a couple of suggestions (gender/region of work), please feel free
