Dear all,
Unfortunately JotForm only allowed me to receive 100 replies/month, so I
look for other options, and I found an option that has unlimited replies,
by now I can only embedded in a site, I can share the code for embedding in
Qgis site. For the meantime I have publish a site
<http://carlosrsuazo.wix.com/qgis>to host the form.
The replies can be exported to Excel.

Please try this new option

Best regards,

Carlos Rivas.
