[Qgis-psc] Grant report: Clean up point cloud index and improve its thread safety (QEP #290)

Dear PSC

I would like to report the results from the grant proposed in QEP #290:
https://github.com/qgis/QGIS-Enhancement-Proposals/issues/290

We have managed to implement all the items as specified in the proposal:
- access to the point cloud index through a shared pointer [4]
- clean up point cloud index API [3]
- unify local/remote implementation of EPT/COPC providers [1], [2]
- clean up hierarchy fetching logic [1], [2]

Thanks to these updates, dealing with point cloud data should be more
robust and better prepared for future use cases.
