Comment (by jef):

 If possible check the "wms_onlineresource" value in the meta data of the
 WEB section of the mapfile (or the response to the GetCapabilities
 request).  I expect you'll find two question marks there as QGIS doesn't
 append a question mark to URIs that already have one.
