 The buttons on the "Create Oracle Connection" dialog are not connected to
 any slots. A new connection can not be created and the dialog cannot be
 closed using the cancel button:

 Warning: QMetaObject::connectSlotsByName: No matching signal for
 on_btnOk_clicked()

 Warning: QMetaObject::connectSlotsByName: No matching signal for
 on_btnCancel_clicked()
