Before you begin, make sure you get the right projections for your map:
Menu Settings > Project Properties > Projection Tab

Use the georeferencer plugin to georef your map
Menu Plugins > Georeferencer

Select your GCPs, at least 4 GCPS are needed to georef your map.  More
is better.

Choose your transformation type (linear/helmert)
