Hi,

QGIS can run offline. Of course you need Internet to download the software.

Certain aspects (optional) require an internet connection. Examples are:

  * Base maps (e.g. OpenStreetmap)
  * Download of additional plugins from the Repository
  * Usage of OGC services (WMS/WFS/WMTS)
  * Some plugins may have additional requirements

Regarding ports: the above mentioned services require port 443 and/or 80 
(normal web ports). If you want to access databases, there are 
additional ports to open (perhaps only on LAN), e.g. for Postgis the 
standard port is 5432 - but you can run it on other ports as well.

Regarding proxy: if your web browser requires a proxy, QGIS will have to 
use the same proxy as well.
