As Giovanni said, it's because those maps just available at some
fixed scale.<br>
<br>
At times when I see a shift in my map like you described, it's
normally because I used the "Zoom in" tool and zoom into an
arbitrary region.<br>
To fix it, I hand-enter the scale at the bottom right box with one
of these values:<br>

OTFR enabled. Sometimes there are "shifts" but if you pan a little the
canvas everything goes in its place (it is because the maps from Google
et al are not available at every scale).
About the error try remove openlayers overview plugin (now integrated
into Openlayers).
