Perhaps it a bit of a quibble, but in American English, what a
report calls a "header" is a "title page". Likewise, what report
calls a "footer" is an "end page".<br>
In common usage, header and footer refer to what appears at the top
and bottom of each page. For example,in a book the even page header
might contain the book title, the odd page header the chapter title,
and the footer would contain the page number. My quick look at the
tutorials seems to suggest there is no equivalent of what are
commonly called header and footer. <br>
This might be the time to change the nomenclature before people get
too used to these terms.<br>
