QGIS displays the SAR file in map coordinates, so it resamples the original slanted radar geometry into a regular square-pixel grid.

That is why the image looks horizontal/vertical.

To keep the original SAR slanted appearance and native pixel spacing, you generally need to view it in its native geometry using SAR software such as ESA SNAP, rather than as a geocoded raster in QGIS.
