Has this image been geo-referenced under the correct CRS indicated on the map? I think is where you should start! Once that is done you will see that if you only open the image the coordinate system will come right. your layers are also correct. For example, if you've set the layer
CRSs to WGS 4326 when they are actually something else, e.g. UTM
