It could also be a scale dependency issue.  The printed map is usually at a difference scale that what you see on the screen.

Is the layer set to transparent in the mapfile?

When I print the map to PDF I only get the top layer in the output.  I don’t have opacity set for any layer.  What am I doing wrong?
