[Journal] Hyperlinking

As part of his review Eli raised some concerns about adding hyperlinks
not submitted by the author. Although it wasn't my intention to
mislead or do anything sneaky, after some thought I agree with his
concerns. In the future I'll work with the authors to create any
hyperlinks in the article text.

I wanted to add something about this to our submission guidelines, and
realized they we didn't have any on the OSGeo wiki. We do now, and
they address the hyperlinking issue:
