hmm. Using firebug, (Net tab), you can look at the requests that to the 
wms server that are being generated. (You will GET requests the wms 
server). The return status might be interesting, but if not, how about 
posting the request that your code is generating from the net tab?
You can test a lot of issues by just fiddling with the request as a URL 
in the browser. That said, I have found ArcGISServer to be major pain in 
the butt. Does it understand EPSG:102067 for starters?
