When speaking about PostGIS database, I would expect that the layer
will be updated when the map is rendered again (can be triggered any
time). But in case you're just putting some custom map items on top of
the map, you can implement that as you wish (e.g. using a timer, maybe
some triggers in postgres?).

Well, it's not a piece of hardware but can be used for transmitting
the location via SMS: it's called BlueSky GPS, a software I've done it
quite some time ago for Symbian phones, Series 60. You can connect it
via Bluetooth to a GPS module, besides of some basic location and
navigation features it's capable of sending SMS with position in
specified interval or sending SMS just on request. Originally it was a
commercial thing, now I don't do anything with it, even the domain has
expired, I'm thinking of releasing it under open source license:
