The scale you see in QGIS is the scale on screen based on the display's
resolution.  The scale the WMS calculates is based on a different fixed
"artificial" resolution.   Therefore the scale QGIS shows is not the
scale the WMS calculates when figuring out what to render.

As neither the scale nor the resolution are specified in the WMS request,
there's not much we can do.   QGIS server was extended to support a
non-standard DPI parameter to solve that.
