[fdo-internals] FDO RFC 45 - Add new FDO PostGIS provider

Hi,

I updated the RFC to say that the new provider will be called OSGeo.PostGISDB to distinguish it from the current OSGeo.PostGIS.

In the future, if the old provider goes away, the new name could stick. We wouldn't necessarily have to rename it.
