Here's what was used for FOSS4G 2007:

The paper name badge inserts for the plastic sleeve
were 4 inches wide, 3 inches tall. There were various
stick-on 'ribbons', for different categories of people.
Some conference feedback forms suggested "Make last names
more visible on name tags, first names are almost meaningless".
The Premier Sponsor package had the option of "Name and/or logo
on the lanyards", provided the sponsor supplied the lanyards.
This option was not used by the sponsor.
The actual cost of "registration badges and tickets" for 2007
was just over 2 dollars per delegate. The tickets were simple
roll tickets used as drink tickets for the Dinner.
From the conference program:
============================
Name Badges
In addition to being a means of identification to colleagues,
you are required to wear your name badge for admission to
conference sessions and social events. Room monitors will
check name badges before allowing anyone in the session or
social event. If you lose your name badge, please visit the
Registration Desk for assistance. Replacement badges will be
issued at a cost of $10.00 CDN per badge.
Green Meetings
FOSS4G has attempted to comply with international Green Meetings
standards to reduce waste at large meetings. We ask your help
in this effort. Please be aware of your use of plastic containers
and reduce consumption wherever possible. Please return name
badges to the Registration Desk and we will happily reuse them.
