I'm not sure that was/is the issue.

Certainly I am not that familiar with GDAL, but I added four lines after
calculating src_win to calculate the ABS of the values, recompiled, and
it fixed my issue.

Thoughts?

David,

If your projwin falls off the edge of the image this can happen.  I have
just patched gdal_translate to terminate if the window is off the image
since things cannot proceed properly.
