Paul,

it is better to not use --with-grass for GDAL but the separate
GDAL-OGR-GRASS Plugin. See for instructions here:
http://grass.gdf-hannover.de/wiki/Compile_and_install_GRASS_and_QGIS_with_GDAL/OGR_Plugin

Then r.out.gdal works nicely.
