Just a comment regarding "Remove continuous build targets that do not support C++11
from Travis-CI and AppVeyor". We'll have to be careful as there are subtle differences in
configurations / drivers tested in some of the non C++11 capable configurations that would
be good to keep. But that should indeed reduce the number of configurations.
