[Geo4All] IMPORTANT and TIME SENSITIVE - correction lab details form

http://osgeo.getinteractive.nl  is the beta website put up by the company
we hired to redo our website. The form is something we are using to collect
information for the beta website, the content will be transferred to our
servers and take over as the main website end of august.

You are correct, I think someone with edit permision was trying to figure
out how it works and kept making new "options 1" entries. I have removed
them.

I am new to google forms we should have the following:

* geoforall lab form: https://goo.gl/forms/SEluzzopSva8N9R13
* resoruce form for course material: https://goo.gl/forms/AfaleJbomJzVmsMH3

Please let me know if those work for you.
