Re 3: that is really goofy. I would have expected the Info or Results tab and that would have meant it did an Inquiry instead of Popups.

Re 1:
Goto the Mapfile and find the appropriate LAYER, then add a TOLERANCE entry and play around with the number until it suits you.  (See MapServer documentation for more information).

Re 2:
Having the column in Postgres and the attribute set as you described should work fine.  You may need to call it "image_url" instead of "url" as "url" may be a reserved MapServer-template word.

Re 3:
That does sound goofy can you attach your mapbook to a follow up post so we can see the service entry for the popups?
