Great to hear this again! Can I suggest
https://github.com/libgeos/geos/issues/472 ("add an optimised
GEOSPreparedDistanceIsWithin method") would be a good candidate here?
That's an API enhancement which could potentially have huge speed
benefits to lots of parts of QGIS (and other GEOS clients).
