about experience with s.db.rim

Hello, There,
        I'm trying to use s.db.rim but failed to create a data
base each time I tried, I got a message of "Make failed due to
Form Definition Errors." I'll really appreciate if any one could
give me an example file for creating a data base in interactive
way or using .make.
