I think that it simply indicates that you GDAL is too old.
As far as I know the version containing LANDSAT-7 FAST support
is not yet released. You will have to compile from GDAL-CVS.
For me LANDSAT-7 import works well with above header files
(note this related bugreport. though:
 http://bugzilla.remotesensing.org/show_bug.cgi?id=208
).
