[Incubator] Code Provenance Review Location

Usually the projects store this where they can all edit it as a group.

You can see some examples (linked to from our wiki) - they range from links
to email threads, through to amazingly detailed reports for the deegree
project.

Personally I liked the one from GDAL as it was easy to follow, and click
through to look at the actual issues turned up.

The GeoServer example is a bit nuts as it was used to organise a team of
volunteers.
